To advance the Prevent Cancer Foundation’s bold goal to reduce cancer deaths by 40% by 2035, the Foundation is proud to support 13 projects dedicated to increasing cancer prevention and early detection through patient navigation across the U.S. and its territories, ranging from Santa Rosa, California to Guaynabo, Puerto Rico to Washington, D.C.
Each program will receive $100,000 over two years. Since 2007, the Prevent Cancer Foundation has awarded over 130 grants to outstanding projects in 38 states, Washington D.C., Puerto Rico, and American Samoa and to the Washoe Tribe. The community grants are generously supported by the Awesome Games Done Quick community and by Barbara Allen in honor of her husband, Ray Allen. Learn more about these diverse projects below.
Canoncito Band of Navajos Health Center, Inc.
ቦታ፡ To’hajiilee, New Mexico Cancers: Breast, cervical and colorectal
Canoncito Band of Navajos Health Center aims to increase colorectal, cervical and breast cancer screening rates. Patient navigators will provide culturally tailored education to help patients access screenings, receive follow-up care and navigate the health care system. They hope to reach 500 people for colorectal cancer screening and 250 women for cervical and breast cancer screening. The project will integrate traditional Navajo values with modern medical practices to improve and increase community cancer prevention awareness and health outcomes.
Health Betterment Initiatives
ቦታ፡ Washington, D.C. Cancer: Liver
Health Betterment Initiatives (HBI) will employ Community Health Workers and Linkage to Care Specialists to provide culturally and linguistically appropriate navigation services, liver health education and hepatitis screenings. HBI aims to engage at least 3,000 people through in-person education and screen at least 1,600 of them. By facilitating early detection through education and screening and guiding people through the health care system, the project aims to reduce liver cancer cases and improve health outcomes in underserved populations in the D.C. metro area.
Horizons Community Solutions, Inc.
ቦታ፡ Albany, Georgia Cancer: ሳንባ
Horizons Community Solutions will provide a comprehensive lung cancer screening and patient navigation program to address the high rates of lung cancer in South Georgia. A dedicated patient navigator will educate patients, identify people eligible for screening, address barriers to screening, ensure appropriate screening tests are ordered and track timely follow-up care across underserved populations. Horizons aims to reach at least 500 people at high risk for lung cancer and facilitate screenings for at least 160 of them.
Jewish Community Free Clinic
ቦታ፡ Santa Rosa, California Cancers: Cervical and colorectal
The “Getting to the Bottom of It” project will target colorectal and cervical cancer prevention and early detection for underserved populations through patient navigation and screening. It will provide education and prevention services through HPV vaccinations and take home fecal test screening kits, as well as follow-up case management and solution-focused therapy for low-income patients who are at greater risk for undiagnosed disease. As part of their two-year project, they aim to train 100 staff, volunteers and students on cancer prevention strategies.
Jewish Family Service of Atlantic & Cape May Counties
ቦታ፡ Margate, New Jersey Cancers: Breast, cervical, colorectal, HPV-related, liver, lung, oral, prostate, skin and testicular
Jewish Family Service of Atlantic & Cape May Counties (JFS), based in southern New Jersey, will provide patient navigation services and health education to boost cancer screenings among clients with serious mental illness (SMI). By the end of the two-year grant period, the goal is for 100 people with SMI to receive their routine cancer screenings and 500 community members to receive educational materials and/or trainings on cancer screening and prevention priorities for those with SMI.
Midwest Asian Health Association
ቦታ፡ Chicago, Illinois Cancer: Liver
The Asian Liver Cancer Prevention Project will promote awareness of liver cancer and prevention among high-risk Asian immigrants and refugee populations through education and linkage to care. The project will facilitate 22 hepatitis B screening and 24 hepatitis B vaccination events and connect medically underserved and uninsured populations who are living with hepatitis B and other liver diseases to health care services to eliminate barriers to screening and stigma of liver cancer.

ቦታ፡ San Juan, Puerto Rico Cancers: Breast and cervical
In collaboration with the Puerto Rico Comprehensive Cancer Center and Puerto Rico Academy of Medical Directors, Migrant Clinicians Network, Inc. (MCN) will launch a two-year initiative to enhance patient navigation for breast and cervical cancer care amidst severe climate disasters, including heat waves, hurricanes and draughts that disrupt health care services. By engaging at least 10 Federally Qualified Health Centers and training approximately 100 patient navigators, the initiative aims to improve cancer care resilience and address structural barriers within historically underserved areas, ensuring continuity of care during climate-related disruptions.
Nevada Cancer Coalition
ቦታ፡ Reno, Nevada Cancers: Breast, cervical, colorectal, lung and skin
ThriveNV-Navigation to Screening is a bilingual patient navigation program providing direct, no-cost navigation services to connect Nevadans with primary care providers and low- or no-cost cancer screening and prevention services. This community-based program will use a supportive and coordinated approach for those facing the most barriers to health care. ThriveNV aims to provide more than 11,000 people with cancer screening information and education and to directly connect more than 300 people to breast, cervical, colorectal, lung and skin cancer screenings.
NewYork-Presbyterian

NewYork-Presbyterian aims to address colorectal cancer (CRC) screening disparities in Manhattan communities that are home to primarily Black, Hispanic and Asian populations. These areas face elevated CRC incidence and mortality rates, exacerbated by significant socioeconomic challenges. NewYork-Presbyterian will employ a CRC patient navigator to provide culturally tailored education, identify and address social determinants of health and aims to support 300 patients in the first year, with the potential to assist more as needed.
One Community Health

One Community Health (OCH) provides primary medical and dental care for roughly 30,000 patients at five clinics in the Pacific Northwest. OCH will implement a focused campaign to increase colorectal cancer screening—particularly in low-income, Hispanic/Latino and Native American communities—by utilizing a team of patient navigators to conduct targeted outreach, host health fairs and educational events, and partner with other community organizations on campaigns around early detection and treatment for colorectal cancer.
VOCES Coalicion de Inmunización y Promocion de la Salud (VOICES Immunization and Health Promotion Coalition)
ቦታ፡ Guaynabo, Puerto Rico Cancers: Cervical and HPV-related cancers
The “VOCES Connect Patient to Care: Breaking Barriers to Care” initiative aims to improve access to cancer care for 200 high-risk people throughout Puerto Rico, focusing specifically on cervical and HPV-related cancers. This project will implement an advanced patient navigation system projected to enhance early detection rates by 50% and decrease mortality rates by 25%. The initiative will engage the community by distributing customized educational content and conducting comprehensive workshops.
Whole Family Health Center, Inc.
ቦታ፡ Vero Beach and Fort Pierce, Florida Cancers: Breast, cervical, colorectal and lung
The Cancer Awareness, Response, and Early Screening (CARES) program at Whole Family Health Center (WFHC) aims to improve cancer prevention and early detection for underserved communities in Indian River and St. Lucie Counties, Florida. Utilizing patient navigation, CARES aims to provide up to 1,200 screenings for breast, cervical, colorectal and lung cancers and reach more than 2,000 WFHC patients through education and awareness efforts.
Woodlawn Health
ቦታ፡ Rochester, Indiana Cancers: Colorectal and pancreatic
Woodlawn Health, a rural critical access hospital in Rochester, Indiana, will launch a mobile cancer prevention screening program to improve early detection among the 20,000+ residents of Fulton County. The project will focus on adults ages 45-80 to provide on-site screenings for various cancers, including breast, cervical, colorectal and lung cancers. A patient navigator will facilitate community outreach, patient scheduling, transportation and follow-up care.
